Try setting <authentication mode="Windows" /> in Web.config... Not sure if this would help...

What are your settings for personalization in web.config?

I've seen this happen with anonymous access. Personalization won't work unless the user is authenticated. You would typically use Windows authentication for an intranet site, and forms authentication for an Internet site.
